| The prostate is a walnut sized
gland at the base of the bladder that encircles the urethra, the tube through
which urine is ejected. The prostate produces prostatic fluid, which makes up
the bulk of the male ejaculate and both nourishes and transports the sperm.
Cancer of the prostate gland is the second leading cause of cancer death among
men. it is primarily a disease of aging. Men in their
30's and 40's rarely develop prostate cancer, but the incidence increases
steadily after 55 years of age. Approximately 80% of all cases occur in men over
the age of 65, and by the age of eighty, 80% of all men have prostate cancer to
some degree. |

The American Cancer Society estimates over 244,000 new cases of prostate
cancer were diagnosed in the year 1995 alone. During that year, 40,400 men died
from the disease. A male baby born today has a 13% chance of developing prostate
cancer at some time in his life, and a 3% chance of dying from the disease. Some
experts believe that every man will eventually develop prostate cancer if he
lives long enough.

| Prostate cancer is a form of cancer that develops in the
prostate, a gland in the male reproductive system. Most prostate cancers are
slow growing; however, there are cases of aggressive prostate cancers. The
cancer cells may metastasize or spread from the prostate to other parts of the
man's body, especially the bones and lymph nodes. Prostate cancer may cause
pain, difficulty in urinating, problems during sexual intercourse, or erectile
dysfunction. Other symptoms may develop during later stages of the disease.
Rates of detection of prostate cancers vary widely across the world, with
South and East Asia detecting less frequently than in Europe, and especially the
United States. Prostate cancer tends to develop in men over the age of 50 and
although it is one of the most prevalent types of cancer in men, many never have
symptoms, undergo no therapy, and eventually die of other causes. |

This is because cancer of the prostate is, in most cases, slow-growing,
symptom free, and since men with the condition are older they often die of
causes unrelated to the prostate cancer, such as heart and circulatory disease,
pneumonia, other unconnected cancers, or old age. On the other hand, the more
aggressive prostate cancers account for more cancer-related mortality than any
other cancer except lung cancer. About 66% of cases are slow growing, the other
third more aggressive and fast developing thus more dangerous.
Thee are many factors such as genetics and diet (poor nutrition) which have
been implicated in the prostate cancer occurring. Prostate cancer might be
indicated by: physical examination, prostate-specific antigen (PSA), or biopsy.
The PSA test increases cancer detection but does not decrease mortality.
Moreover, prostate test screening is controversial at the moment and may lead to
unnecessary, even harmful, consequences in some patients. And suspected prostate
cancer is typically confirmed by taking a biopsy of the prostate and examining
it under a microscope. Further tests like CT scans and bone scans, can be
performed to determine whether the man's prostate cancer has spread.
Treatment options for prostate cancer with intent to cure are primarily
surgery, radiation therapy, stereotactic radiosurgery, and proton therapy. Other
treatments, such as hormonal therapy, chemotherapy, cryosurgery, and high
intensity focused ultrasound (HIFU) also exist, but may not be FDA approved,
depending on the clinical scenario and desired outcome.
The man's age and general health considered with the extent of metastasis,
appearance under the microscope, and response of the cancer to initial treatment
are important in determining the outcome of the disease. The decision whether or
not to treat localized prostate cancer that is a tumor that is contained within
the prostate with curative intent is a patient trade-off between the expected
beneficial and harmful effects in terms of patient survival and their quality of
life.
