Signs and symptoms - many organs can be affected by
myeloma so the symptoms and signs vary greatly. A mnemonic sometimes used to
remember the common tetrad (4 parts) of multiple myeloma is CRAB: C = Calcium
(elevated), R = Renal failure, A = Anemia, B = Bone lesions. Myeloma has many
possible symptoms, and all symptoms may be due to other causes. They are
presented here in decreasing order of incidence.
Bone Pain - myeloma bone pain usually involves the spine
and ribs, and worsens with activity. Persistent localized pain may indicate a
pathological bone fracture. Involvement of the vertebrae may lead to spinal cord
compression. Myeloma bone disease is due to the over expression of Receptor
Activator for Nuclear Factor (RANKL) by bone marrow stroma. RANKL activates the
osteoclasts that resorb bone. The resulting bone lesions breakdown and are seen
in plain radiographs, which may show "punched-out" lesions including
the "pepper pot" appearance of the skull on radiography. The bone
breakdown also releases calcium into the blood thus leading to hypercalcemia and
associated symptoms.
Anemia - common with in multiple myeloma is usually
normocytic and normochromic, resulting from the replacement of the persons
normal bone marrow by infiltrating tumor cells and inhibition of normal red
blood cell production "hematopoiesis" by cytokines.

Myeloma cells often collect in the
bone marrow and in the outer part of bones. They may collect in only one bone
and form a single mass, or tumor, called a plasmacytoma. But in most cases, the
myeloma cells collect in many bones, forming many tumors and causing numerous
problems. When this occurs, the disease is called multiple myeloma.
Although and multiple myeloma and plasmacytoma do affect the
bones, they start in immune system cells. This type of cancer is different from bone
cancer, which begins in cells that form the hard, outer part of the bone.
This is important because the diagnosis and treatment of plasmacytoma and/or
multiple myeloma is different from then that of bone cancer.
According to the International Myeloma Foundation, there are
over 13,500 new cases of Myeloma in the United States every year, representing
20% of blood cancers, and 1% of all forms of cancer.
Myeloma is more common in blacks that whites and men get it more then women. The
risk of the disease increases with age. Better diagnostic techniques and the
higher average age of the general population may explain the rising incidence of
myeloma over the past several decades. More patients under 55 years old are
being diagnosed with myeloma; risk factors include: airborne particles, atomic
radiation exposure, petroleum products, pesticides, solvents, heavy metals, and
industrial chemicals.
Treatment may include: Fractionated dose chemotherapy, bone
marrow transplant, autologous stem cell rescue with post transplant
immunotherapy, and of course nutritional and herbal supplementation.
