High cholesterol or hypercholesterolemia is the presence of high levels of cholesterol in the blood of a person. And it is closely related to the medical terms hyperlipidemia elevated levels of lipids in the blood and the term  hyperlipoproteinemia elevated levels of lipoproteins in the blood.

Elevated cholesterol in the blood is normally caused by abnormalities in the levels of lipoproteins, the particles that carry cholesterol in the bloodstream. This may be related to diet, genetic factors (like LDL receptor mutations in familial hypercholesterolemia) and the presence of other diseases such as diabetes and an underactive thyroid called hypothyroidism. The type of hypercholesterolemia depends on which type of particle like a low density lipoprotein happens to be present in excessive amounts.

High cholesterol treatment includes reducing dietary cholesterol intake, administration of certain drugs medications, but rarely with other treatments like surgery.

High cholesterol is normally caused by a combination of environmental and genetic factors. Environmental factors include: obesity and poor dietary choices. Genetic contributions are normally due to the additive effects of multiple genes but sometimes can be due to a single gene defect such as familial hypercholesterolaemia. A number of secondary causes exist including: diabetes mellitus type 2, obesity, alcoholism, monoclonal gammopathy, dialysis, nephrotic syndrome, obstructive jaundice, hypothyroidism, Cushing’s syndrome, anorexia nervosa, medications such as thiazide diuretics, ciclosporin, glucocorticoids, beta blockers, retinoic acid.

While part of the circulating cholesterol originates from diet, and restricting cholesterol intake can help reduce blood cholesterol levels there are other links between the dietary pattern and cholesterol levels.

 

 

 

 

 

 

Genetic abnormalities maybe in some cases completely responsible for High cholesterol, such as in familial hypercholesterolemia where there is one or more genetic mutations in the LDL receptor. Even when no single responsible mutation to explain hypercholesterolemia is known a genetic predisposition still plays a major role. Potentially adding to poor diet and lifestyle factors to multiply the risk of late complications. Multiple genes are involved, and high cholesterol where there is likely a genetic predisposition is called polygenic hypercholesterolemia. Where the involved genes have yet to be identified.[

There is no specific cutoff line between normal and abnormal cholesterol levels and interpretation of values needs to occur in relation to other health factors. An ideal cholesterol level would be less than 4 mmol/l with a LDL cholesterol and less than 2 mmol/l in those people at high risk of cardiovascular disease.

Higher cholesterol levels also increase the risk of several diseases especially cardiovascular diseases. And specifically high levels of small LDL cholesterol particles are related to increased risk. However, larger LDL particles may not carry the same risk.

When cholesterol levels are measured, it is important to measure its subfractions (LDL, HDL and VLDL) before drawing a conclusion as to the cause of the problem. In the past, the LDL and VLDL levels were rarely measured directly due to the high cost of testing. The VLDL levels are reflected in the levels of triglycerides (about 45% of triglycerides are composed of VLDL). The LDL was normally estimated from the other fractions using the following equation (LDL \ approx total cholesterol - HDL - (0.2 x triglycerides)) and this formula is called the Friedewald calculation
