| Fibroids also called "uterine
myomas" are growths that form on the interior muscular wall or exterior
of the uterus. They are almost never malignant. The problem may also affect
the cervix. It is estimated that 20 to 30% of women get fibroids and asian &
african american women are at greater risk. The fibroids usually occur between
the ages of 32 to 46 and normally shrink after menopause.
It is believed that the hormone estrogen is involved and the problem maybe
genetically passed in the family. The fibroids may cause heave and frequent
menstrual periods. They can cause infertility,
anemia, bleeding between periods, fatigue,
and increased vaginal discharge. Fibroids also may be a cause of painful sexual
intercourse or cause bleeding to occur after sexual intercourse. This may
play a role in sexual dysfunction for

Other problems that may occur depending on where the
fibroids are located as bowel pain or pressure, bladder problems, and urethra or
kidney blockages. Use of oral contraceptives may increase the risk of fibroids.

A uterine fibroid also known as: uterine leiomyoma, myoma, fibromyoma,
leiofibromyoma, fibroleiomyoma, and fibroma.
| The signs and symptoms of fibroids especially when smaller, can
be entirely asymptomatic. Symptoms depend on the location of the lesion and its
size. The important symptoms include: abnormal gynecologic hemorrhage, heavy or
painful periods, abdominal discomfort, bloating, painful defecation, back
ache, urinary frequency or retention, and in some cases, infertility.
There may also be pain during intercourse, which depends on the location of the
fibroid(s). During pregnancy they can be the cause of miscarriage, bleeding,
premature labor, or interfering with the position of the fetus.
While fibroids are common in women especially blacks and asians, they may or
may not be related to infertility accounting for about 3% of reasons why a woman
may not have a child. Normally in such cases a fibroid is located in a
submucosal position and it is thought that this location may interfere with the
function of the lining and the ability of the embryo to implant. Also larger
fibroids may distort or block the fallopian tubes so that conception may be
affected.
The growth and location are the major factors determining if a fibroid will
lead to serious symptoms and health problems. A small lesion can be symptomatic
if located within the uterine cavity while a large lesion on the outside of the
uterus may go unnoticed. Different locations are classified as follows:
* Intramural fibroids - are located within the wall of the woman's
uterus and are the most common type and unless they are large, they may be
asymptomatic. Intramural fibroids start as small nodules in the muscular wall of
the uterus. And in time, intramural fibroids can grow inwards causing distortion
and elongation of the uterus..
* Subserosal fibroids - are located underneath the mucosal (or peritoneal
area of the uterus and they may become very large. They may also grow out in a
papillary manner to become what are known as pedunculated fibroids and these
type growths can actually detach from the uterus and become a parasitic
leiomyoma.
* Submucosal fibroids - are located in the muscle under the endometrium
of the uterus and they will distort the uterine cavity; even small lesion in
this location can cause bleeding and also infertility. A pedunculated lesion
within the cavity is termed an intracavitary fibroid and can be passed through
the cervix.
* Cervical fibroids - are located in the wall of the cervix the
"neck of the uterus". However, rarely are fibroids found in the
supporting structures such as round ligament, broad ligament, or uterosacral
ligament of the uterus that also have the smooth muscle tissue.
Fibroids may be single or multiple. Most fibroids start in an intramural
location, that is the layer of the muscle of the uterus, and with more growth,
some lesions may develop towards the outside of the uterus or towards the
internal cavity. Secondary changes that may develop within fibroids are
hemorrhage, necrosis, calcification, and also cystic changes. |
