| Cardiomyopathy involve physical
changes in the heart such as enlargement or dilation of one or more of the
chambers of the heart. It can also involve rigidity of the heart muscle. The
disorder maybe related to inherited defects or can be caused by a number of
different diseases. Often the cause is unknown. Cardiomyopathy
is difficult to treat. Beta-blockers such as Metoprolol "Lopressor"
or Propranolol "Inderal" may help in some cases by slowing the
heart rhythm and allowing better filling of the heart chambers between beats.
Fluid retention may be treated with diuretics. Ultimately, heart
transplantation may be the only option. However, the cost, limited availability,
trauma of massive surgery, and the suppression of the body's immune system
caused by the anti-rejection drugs needed after surgery make this the very last
resort.

| Symptoms and signs may look like those of almost any
other type of heart disease. Chest pain is common. Mild myocarditis or
cardiomyopathy is frequently asymptomatic; severe cases are associated with
heart failure, arrhythmias, and
systemic embolization. Manifestations of the underlying disease Chagas'
disease may be prominent. Many patients with biopsy-proven myocarditis
report a recent viral prodrome preceding their cardiovascular symptoms.
And EKG abnormalities are often present, although the changes are often
nonspecific. A pattern characteristic of left ventricular hypertrophy may be
present with flat or inverted T waves being the most common, often with
low-voltage QRS complexes. Intraventricular conduction defects and bundle branch
block, especially left bundle branch block, are also common. An echocardiogram
is useful to detect wall motion abnormalities or a pericardial effusion. Chest
radiographs may be normal or may show some evidence of congestive heart failure
with pulmonary edema or cardiomegaly. |

Cardiomyopathies can be either extrinsic or intrinsic.
Extrinsic cardiomyopathy - is a cardiomyopathy in which the primary
pathology is outside the myocardium itself. And most cardiomyopathies are of the
extrinsic type, because by far the most common cause of a cardiomyopathy is
ischemia. The World Health Organization calls these specific cardiomyopathies.
Intrinsic cardiomyopathy is defined as weakness in the muscle of the
heart not due to an identifiable external cause. And this type definition was
used to categorize previously idiopathic cardiomyopathies although specific
external causes have since been identified for many. For example, alcoholism
has been identified as a cause for some forms of dilated cardiomyopathy. To make
a diagnosis of an intrinsic cardiomyopathy, significant coronary artery disease
should be ruled out. The medical term intrinsic cardiomyopathy does not
describe the exact etiology of weakened heart muscle. The intrinsic
cardiomyopathies consist of a variety of disease states, each with their own
causes. Many intrinsic cardiomyopathies now have identifiable external causes
including drug and alcohol toxicity, certain infections such as Hepatitis
C and various genetic and unknown idiopathic causes. Also it is
possible to classify cardiomyopathies functionally, as involving dilation,
hypertrophy, or restriction
